Position Description

ORGANIZATION: Citizen Care Inc.
DEPARTMENT: Life Sharing
POSITION: Life Sharing Medical Coordinator
STATUS: Full-Time; 40 hours / week
SCHEDULE: Flexible to meet the needs of the program (primarily daylight)
LOCATION: Based in Robinson Twp., but travel to program sites within Allegheny, Washington, Beaver, and other neighboring counties. 

Citizen Care Inc., a subsidiary of Partners For Quality Inc., provides residential, day program, habilitation, supported employment opportunities and behavioral support services to adults with intellectual and developmental disabilities. We are a non-profit organization, accredited by the Council on Quality & Leadership. Citizen Care has locations throughout the West Hills and South Hills of Allegheny County.

The Life Sharing Medical Coordinator serves as a member of the Community and Employment Supports Services leadership team reporting to the Director of Life Sharing. The Life Sharing Medical Coordinator is responsible for providing and overseeing medical care to adults and children with intellectual disabilities and autism who receive support in both typical and medically complex (MCC) life sharing programs as well as people supported in the Family Medical Support Assistance (FMSA) program. The Life Sharing Medical Coordinator provides oversight in all aspects of the Health Risk Screening Tool (HRST) implementation to promote a better quality of life for all people supported by Citizen Care. Responsibilities will include assisting in coordinating services to support the medical, health, and wellness needs for people supported. The Life Sharing Medical Coordinator will bring a positive and person-centered care approach and work cooperatively with administrative and program staff. This position will require travel to the homes of people supported for home visits and will have the flexibility to telework from home on a Citizen Care device as well as in the administrative office when not in the field as scheduled with the Director of Life Sharing.

Health & Wellness

  • Demonstrate a working knowledge and understanding of personal outcomes for people supported, especially as they apply to their health, wellness, and medical needs.
  • Advocate for people supported to achieve their health and wellness goals through education and consultation.
  • Guide and assist Life Sharing staff, Life Sharing Partners, and families receiving FMSA to support the attainment of the best possible health and personal outcomes for people supported.
  • Provide oversight and direction for the implementation/integration of the HRST, implement service and training considerations as indicated by the HRST, and ensure routine up-dates and proper documentation within the screening tool as required.

Administrative Duties & Professional Standards

  • Complete and maintain certification as an HRST rater and clinical reviewer.
  • Serve as the clinical reviewer for completed HRSTs within the life sharing programs.
  • Demonstrate familiarity with Office of Developmental Programs’ (ODP) Medication Administration Process, including successful completion and maintenance of train the trainer certification; review medication errors when needed.
  • Collaborate with the Life Sharing team to determine the appropriateness of referrals in both Life Sharing and FMSA programs; conduct initial meetings with the people referred and their families/caregivers, ensure receipt of required information as necessary.
  • Complete and maintain certification as a First Aid/CPR trainer.

Strategic Leadership

  • Display a strong commitment to the accomplishment of CCI’s Mission and Vision.
  • Support and actively model values associated with The Council on Quality & Leadership (CQL) and Everyday Lives.
  • Work collaboratively with the CCI team to develop strategies to proactively close identified care gaps (HEDIS).
  • Identify, track, and analyze health and wellness trends/outcomes utilizing data from HRST and other associated health and wellness programs in place at CCI.
  • Explore collaboration with other medical and/or health and wellness focused facilities or agencies to support CCI programs and services.
  • Responsible for the completion of other related duties as assigned.

EOE

  1. Must be a Registered Nurse (RN) with a current Pennsylvania license
  2. Four (4) years experience in nursing required
  3. Experience in Intellectual Disabilities, Autism, and Mental Health Services preferred
  4. Experience in dealing with emergency and/or acute medical situations preferred
  5. Valid PA driver’s license
  6. Use of personal vehicle required
  7. Act 33 / 34 and FBI Clearances
  8. Excellent references, both personal and work
