Position Description

We are currently seeking a LPN to join our team! As a Licensed Practical Nurse (LPN) in a PACE program, you will work within an Interdisciplinary Team (IDT) to provide comprehensive care to elderly Participants, promoting their health and independence in the community. You will implement nursing care plans, conduct assessments, provide treatments, and offer education and counseling to Participants and their families. Additionally, you will oversee and mentor health aides to ensure high-quality care delivery.

Job Title: Licensed Practical Nurse (LPN)

Pay: $53,000 – $76,000 annually, based on experience and qualifications.

Schedule:

  • Work Hours: Monday – Friday, 8:00 AM to 4:30 PM (no weekends, no evenings!)
  • On-Call: Likely 1-2 holidays per year; but there is a separate on-call nursing team covering most regular on-call shifts.

Location(s):

The PACE program operates several Adult Day Health Centers (ADHC) around the Pittsburgh area. The most urgent need is at the McKees Rocks location, but we are hiring for all centers:

  • Greentree
  • North Shore
  • Allegheny County
  • McKees Rocks

Please Note: Each center serves a caseload of approximately 200 Participants. Travel between centers is uncommon, but nurses may need to visit Participants in various settings as needed.

Essential Functions:

  • Commitment to Mission: Demonstrates a strong commitment to supporting elderly individuals in maintaining their dignity and independence within the community.
  • Vital Signs: Take and monitor Participant vital signs, including respiration, temperature, blood pressure, and heart rate.
  • Direct Nursing Care: Administer basic nursing care including wound care, dressing changes, catheter insertion, respiratory treatments, and other tasks based on individual skills/credentials.
  • Communication with Medical Team: Report Participant status to the Registered Nurse (RN), Primary Care Physician (PCP), and/or Clinical Nurse Practitioner (CRNP).
  • Medication Administration: Administer medications in the center, verify pharmacy counts, prepare medication boxes, and deliver medications to Participant homes.
  • Diagnostic Tests: Perform basic diagnostic tests such as PT INR, blood glucose, and urine dip tests. Report results to the RN/CRNP/PCP.
  • Lab Requisitions: Manage and process laboratory requisitions.
  • Physician Orders: Assist with the physician order process.
  • Professionalism & Customer Service: Provide services with courtesy, patience, and professionalism, representing the PACE program in a positive manner to Participants, families, co-workers, vendors, and the general public.
  • Compliance: Maintain the necessary certifications and comply with educational and health requirements (e.g., annual TB tests, bi-annual physical exams, fire safety training, etc.).

Working Conditions:

  • Primarily based in PACE Adult Day Health Centers (ADHC), but may travel to Participant residences, hospitals, Skilled Nursing Facilities (SNFs), or other settings as needed.
  • Potential exposure to unpleasant sights, smells, bloodborne pathogens, or communicable diseases. Standard precautions must be utilized at all times.

Knowledge, Skills & Abilities:

  • Patient Care: Ability to assess and interpret information to address the specific needs of Participants.
  • Attention to Detail: Must be able to manage multiple details in a fast-paced environment.
  • Anatomy & Treatment: Knowledge of human anatomy and the treatment needs of elderly Participants.
  • Communication Skills: Strong verbal and written communication skills, with the ability to collaborate effectively with interdisciplinary team members.
  • Problem-Solving: Ability to assess complex situations, provide solutions, and manage crises when necessary.
  • Cultural Sensitivity: Ability to work effectively with people from diverse backgrounds.

Flexibility & Independence: Must be adaptable and able to work independently as well as part of a team.
